Alumni of most schools can order and pay for official transcripts online through the National Student Clearinghouse (NSC). Processing times will vary and are in addition to delivery times. Please see below for more information about placing an order and delivery options.

As of Spring 2022, all careers are now presented in one academic transcript in order of program level. We can no longer issue separate transcripts by career. The appearance of the official University transcript will vary depending on the system of record at the time of attendance.

Forms, Holds, and Third-Party Requests

Forms and Identification Required by Transcript Processing Centers

A matching form or identification number is used by some transcript processing centers to match a graduate or professional school applicant's transcript with their application. Most forms or ID numbers can be submitted online with your order and delivered along with your official transcript.

Please not that the form attachment review process can take 1-3 business days.

Removing Holds on a Student Record

If you are on a hold you may not have access to order an official transcript or be able to view your grades or unofficial transcript on Path@Penn. Please note that grades and GPAs will not be disclosed over the phone, via email, or in person. Assistance with clearing a hold on your record is dependent on the type of hold. For more information about holds and how to clear them, please visit the Holds on Student Records & Accounts page.

Requesting Release of Records by Special Investigators

We strongly encourage Special Investigators and other government employees to use DoD Safe when requesting transcripts. DoD Safe links should be sent to registrar@upenn.edu. Release forms should not be directly emailed as they may contain Personal Identifiable Information (PII) such as Social Security Numbers (SSN).

If your unit does not use DoD Safe, please email registrar@upenn.edu for support.

Penn Nursing Affiliates

Students who graduated from St. Joseph’s Hospital School of Nursing, Lancaster, PA, Episcopal Hospital School of Nursing RN & LPN programs (Temple University Health System), Episcopal School of Perfusion Science (Temple University Health System), or Northeastern Hospital School of Nursing (Temple University Health System) should contact the Barbara Bates Center for transcripts.

For other schools and programs:

  • Albert Einstein Medical Center School of Nursing graduates should contact the Albert Einstein Medical Center Education Department at (215) 456-1130.
  • Graduates of the Bryn Mawr Hospital School of Nursing should call (484) 337-8596 to request their transcripts
  • Chestnut Hill Hospital School of Nursing graduates should contact Evelyn Vanish at 215-248-8917 or EvelynEmily.Vanish@towerhealth.org for their records.
  • Graduates of the Hospital of the University of Pennsylvania (HUP) School of Nursing should contact the following to receive their transcripts and/or other records: HUP School of Nursing Transcripts, Human Resources Records Section, 3930 Chestnut Street, Philadelphia, PA 19104, (215) 615-2625 (pre-1978).
  • All archival materials from the Pennsylvania Hospital School of Nursing are housed at the Historic Pennsylvania Library and Archives. For more information, please contact Stacey Peeples, Curator & Lead Archivist at 215-829-5434
  • Graduates of the Philadelphia General Hospital School of Nursing should contact the Philadelphia City Archives to obtain information about ordering transcripts.
  • Graduates of the Presbyterian Hospital School of Nursing RN and LPN programs should contact the Nursing Education offices at the Penn Presbyterian Medical Center at 215-662-9161.
  • Graduates of the University of Pennsylvania School Of Nursing’s BSN, MSN, and PhD programs must request their transcript from the Office of the University Registrar.
