Path@Penn provides students with simple forms they can use to make requests related to their academic programs and student records. The Office of the University Registrar maintains and processes these forms in partnership with your school.
If you are an advisor and need to review a form your student has submitted, you can do so by logging in to BP Logix and reviewing your My Tasks dashboard.
Access to these forms does not infer approval for processing. All submissions are reviewed by the appropriate office prior to processing.
Academic Program Requests

Declare/Update Field of Study | Students | Request to declare or update your major, minor, or concentration. |
Certificate Request | Students and Administrators | Request to add or drop an in-program certificate. |
Max C.U. Increase | Students and Administrators |
Request an increase to the number of credits for which you can register in a given term. |
Withdraw From a Course | Students | Request to withdraw from a course. |
Grade Mode Change (formerly known as Grade Type) | Students |
Request to change a course to pass/fail or to standard grading options. |
Internal Transfer/Dual Degree | Students and Administrators |
Students: Request to separate from a dual-degree program. Administrators: Submit a request on behalf of a traditional undergraduate for an internal transfer or dual-degree program |
Drop Below Required Enrollment | ISSS Administrators | ISSS administrators can submit a request to update a F1 or J1 visa student’s minimum CUs |
Non-Course Requirements | Administrators | Record required academic benchmarks for non-PhD, non-EdD, non-MSEd, and non-Professional Masters students |
Update Expected Graduation Date | Administrators | Update the graduation date for any active program on a student's record. |
Add Masters for PhD Student | Students and Administrators |
Request to add a research master's degree (MA or MS) for PhD students ONLY. Can also be used when a research master's is already on a PhD student's record but unable to apply for graduation via the Graduation Application. |

Student Record Data

Student Data Update | Students | Request an update to your critical data such as name, social security number, marital status, date of birth, visa or citizenship status. |
FERPA Opt-Out | Students and recent alumni | Manage your privacy setting related to your directory information with third parties. Be sure to read the details to understand what opting out means for you. |
FERPA Opt-Out Form (PDF) | Pre-2010 Alumni | Submit a request form (PDF) for Penn to withhold directory information from third parties. |

Transcript Forms

Transcript Request Form | Students or alumni | Paper transcript request form to be used only by those who cannot submit a request on Penn InTouch. |
