Path Forms

Path Forms provide the ability for administrators and students to request changes and exceptions related to student academic and financial records. The Office of the University Registrar (OUR) maintains and processes these forms in partnership with the schools.

Access to Path forms does not infer approval for processing. All submissions are reviewed by the appropriate office before processing.
