Path Forms

Path@Penn provides students with simple forms they can use to make requests related to their academic programs and student records. The Office of the University Registrar maintains and processes these forms in partnership with your school.

If you are an advisor and need to review a form your student has submitted, you can do so by logging in to BP Logix and reviewing your My Tasks dashboard. 

Access to these forms does not infer approval for processing. All submissions are reviewed by the appropriate office prior to processing.

Academic Program Requests

Form Name Audience Form Description
Declare/Update Field of Study       Students                                               Request to declare or update your major, minor, or concentration.
Certificate Request Students Request to add or drop an in-program certificate.
Max C.U. Increase College, SEAS, and Wharton undergraduates; all LPS and Nursing students

Request an increase to the number of credits for which you can register in a given term.

Withdraw From a Course Students Request to withdraw from a course.
Grade Type Change Students Request to change a course to pass/fail or to standard grading options.
Internal Transfer/Dual Degree Students and Administrators

Students: Request to separate from a dual-degree program.

Administrators: Submit a request on behalf of a traditional undergraduate for an internal transfer or dual-degree program

Student Record Data

FERPA Opt-Out Form Students To request that Penn not share certain directory information with third parties. 

Leaves and Returns

Request for Separation School administrator                           Initiate a separation (leave of absence, withdrawal) request on behalf of a student.

Graduate Forms

Transcript Forms

Transcript Apostille Seal Application        Students or alumni                                           Request an apostille or state seal on your transcript.
Transcript Request Form Students or alumni      Paper transcript request form to be used only by those who cannot submit a request on Penn InTouch.
