Academic Program Requests

Request an increase to the number of credits for which you can register in a given term.

Grade Mode Change (formerly known as Grade Type) Students

Request to change a course to pass/fail or to standard grading options.

Internal Transfer/Dual Degree Students and Administrators

Students: Request to separate from a dual-degree program.

Administrators: Submit a request on behalf of a traditional undergraduate for an internal transfer or dual-degree program

Drop Below Required Enrollment ISSS Administrators ISSS administrators can submit a request to update a F1 or J1 visa student’s minimum CUs
Non-Course Requirements Administrators Record required academic benchmarks for non-PhD, non-EdD, non-MSEd, and non-Professional Masters students
Update Expected Graduation Date Administrators Update the graduation date for any active program on a student's record.
Add Masters for PhD Student Students and Administrators

Request to add a research master's degree (MA or MS) for PhD students ONLY. 

Can also be used when a research master's is already on a PhD student's record but unable to apply for graduation via the Graduation Application.

Change Program Priorities Administrators Request to change program priorities for a student enrolled in at least two programs.

FERPA Opt-Out Form (PDF) Pre-2010 Alumni Submit a request form (PDF) for Penn to withhold directory information from third parties.

Transcript Forms
